
A federal safety agency has issued a final report into a deadly explosion at U.S. Steel's Clairton Coke Works facility near Pittsburgh that killed two workers and injured 11 others in August 2025. According to reports from the Associated Press, the blast occurred when workers were flushing a gas valve with pressurized water in preparation for routine planned maintenance at the sprawling riverside industrial facility, which is the largest of its kind in the Western Hemisphere. The valve ruptured under intense pressure and released combustible coke oven gas, resulting in the fatal incident. Twenty-four seconds after a radio call went out to evacuate the area at 10:47 am, gas rushing out of the failed valve ignited, destroying the building and seriously injuring several workers. The explosion caused ₹4,320 lakh ($52 million) in damage. Timothy Quinn, 39, of South Huntingdon and Steven Menefee, 52, of East Huntingdon were killed in the blasts, with first responders from 14 fire departments and 20 EMS companies spending hours searching the 293-acre property for survivors.
The U.S. Chemical Safety and Hazard Investigation Board concluded that U.S. Steel's policies, procedures, and practices for managing process safety were ineffective, making the explosion preventable. As reported by the Associated Press, the board found that U.S. Steel lacked effective safety management systems that could have prevented or reduced the severity of the explosions. The report stated that 'this deadly incident was the result of an ad hoc informal procedure, poor facility siting and an ineffective process safety management system at the Clairton facility.' The board recommended that U.S. Steel develop a written procedure for washing valves with pressurized water to minimize operation hazards, and establish a corporate safety governance program to implement safety management at its facilities. Chemical Safety Board member Sylvia Johnson stated that the blast 'was the result of workers routinely performing a task incorrectly over a period of years until it ultimately led to a catastrophic explosion.' The board also recommended that an industrial cleaning contractor which used a pump truck to apply pressurized water to the valve develop written policies and procedures for cleaning piping systems containing flammable or toxic gas, and train its workers in such operations.
The failed valve was more than 70 years old and made from cast iron, which the report noted is widely recognized as a brittle material. As reported by the Associated Press, numerous safety publications either ban or warn against using cast iron equipment in certain hazardous applications, including for flammable substances such as coke oven gas. The report found that the two workers who died and two of the five seriously injured workers were located around buildings directly above the coke oven gas piping. U.S. Steel operated its facility while routinely occupied non-blast resistant buildings were located less than 20 feet directly above a hazardous piping system, with the board noting that the company did not conduct a siting analysis despite industry recommendations. The faulty valve was located between two coke oven batteries, Battery 13 and Battery 14, where workers were conducting maintenance on valves that regulate coke oven gas flow. The report also noted that a similar gas oven explosion occurred at the Clairton plant in July 2010, injuring 14 U.S. Steel employees and six contractors, with the board criticizing U.S. Steel for failing to conduct a subsequent facility siting evaluation after that incident.
The Occupational Safety and Health Administration previously fined U.S. Steel a total of ₹9.5 lakh ($118,214) and issued 10 citations following the incident. According to the Associated Press, OSHA determined that U.S. Steel's written procedures, practices and training to safely maintain equipment and flush gas valves were incomplete, outdated or inadequate. The agency found that employees had utilized pressurized water to clean valve seats on an ad hoc basis for at least three years before the incident despite the lack of formal procedures. MPW Industrial Services, Inc., a contractor for U.S. Steel, was fined an additional ₹5,100 lakh ($61,000) in the incident, with both companies contesting the citations. OSHA also hit MPW Industrial Services with 9 violations and $61,000 in fines, mainly tied to procedure and training issues. The report revealed that when a valve would not close fully, U.S. Steel had a practice of injecting steam or high-pressure water into the cleanout port in an attempt to remove residue, with the board noting that 'U.S. Steel had no procedure for how to safely conduct this operation.'
The United Steelworkers union, which represents the plant's 1,100 unionized workers, has committed to pushing for better safety at U.S. Steel plants. United Steelworkers president Roxanne Brown stated that 'No family should ever have to wonder whether their loved one will make it home safely from work,' while Steelworkers' District 10 Director Bernie Hall emphasized that 'Safety requires constant vigilance and a shared commitment.' U.S. Steel spokesperson Amanda Malkowski confirmed that the company has strengthened several safety protocols, with multi-disciplinary teams collaborating to establish standardized best practices for industrial valve cleaning. The company released a statement commemorating the one-year anniversary, stating that safety 'shapes every part of our business' and expressing commitment to ensuring safety remains at the center of operations. However, the report noted that before the blast, Clairton Coke Works had a history of accidents and explosions, with some current and former workers saying poor management and underinvestment have exacerbated air pollution and undermined workplace safety. The blast came on top of a string of other accidents at the Clairton plant over time, as well as a long history of legal battles between U.S. Steel and Allegheny County regulators, who regularly accuse the company of flouting environmental rules at the facility.
